This will involve both clinical responsibilities of managing a small caseload as well as day to day operational responsibilities and will require the post holder to deputise for the clinical lead as and when appropriate/required.Key Duties To support the clinical lead in the implementation of integrated care pathways including appropriate service user assessments and models of care within a stepped care mental health model. Co-ordinate care delivery, across the mental healthcare pathway e.g. referrals management, allocations of named worker/care co-ordinator, ensuring seamless continuity of care across the stepped care model dependent on service user need.
