      <description>Location: Exeter | Salary: 45000.00-45000.00 Annual | Type: Permanent | We are recruiting for an experienced Clinical Case Manager to join a specialist community-based complex care service in Exeter. This is an excellent opportunity for a Registered Nurse with strong community complex care experience who can provide clinical leadership, support care packages and work closely with clients, families, staff and external professionals. Key Responsibilities  Assess clients'' physical and mental health needs. Develop personalised, outcome-focused care plans. Complete clinical and environmental risk assessments. Review care packages in response to changing needs. Provide clinical support to clients and their families. Act as a key communication link between families, professionals and care teams. Support the identification and induction of staff into complex care packages. Provide ongoing clinical support to nurses and care staff. Complete staff supervisions, appraisals and competencies. Support training and development of clinical and non-clinical staff. Interview healthcare staff. Work with commissioners to support new care packages.
