Build and maintain effective relationships with GP practices and partner agencies. Manage referrals and maintain accurate case records. Conduct risk and needs assessments for individuals experiencing domestic abuse. Provide advocacy, safety planning, emotional support and practical advice to service users. Support clients to access appropriate services, including safeguarding, IDVA and MARAC referrals where required. Attend multi-agency meetings and represent the service professionally. Raise awareness of domestic abuse and contribute to newsletters, reports and service development. Collect and analyse referral and outcome data for reporting purposes.
