The main focus of this role is working with people fleeing domestic abuse who may have complex support needs. You will be responsible for coordinating referrals into the service, working closely with professionals, partner agencies and the wider support team to ensure people are assessed and supported appropriately. Main duties will include but not limited too; Coordinate and manage referrals into the service. Liaise with referring agencies, professionals and partner organisations. Gather and assess referral information to identify support needs and risks. Complete initial risk assessments and ensure relevant information is shared appropriately. Coordinate placements and liaise with the team regarding accommodation availability. Maintain accurate referral records, case notes and databases.
