Referrals Coordinator (Vulnerable Adults)

BDS Recruitment are currently recruiting for an organised and experienced Referral Coordinator to join a supported housing service from either their Tolvaddon or St Austell office. The main focus of this role is working with people fleeing domestic abuse who may have complex support needs. You will be responsible for coordinating referrals into the service, working closely with professionals, partner agencies and the wider support team to ensure people are assessed and supported appropriately. Main duties will include but not limited too; Coordinate and manage referrals into the service. Liaise with referring agencies, professionals and partner organisations. Gather and assess referral information to identify support needs and risks. Complete initial risk assessments and ensure relevant information is shared appropriately. Coordinate placements and liaise with the team regarding accommodation availability. Maintain accurate referral records, case notes and databases. About you You will ideally have experience in referrals, supported housing, homelessness, domestic abuse, housing, social care, safeguarding or community services. Previous experience in a coordination, referral, housing or support-based role. An understanding of risk assessment and safeguarding.
