Referral and Assessment Coordinator Immediately Available Opportunity Comfort Care Services Slough HQ, Berkshire Permanent Full-time Monday to Friday, 9.00am to 5.00pm About the opportunity We at Comfort Care Services are looking for an experienced Senior Administrator to join our Assessments Team as a Referral and Assessment Coordinator. You will take ownership of the administration supporting each referral from initial enquiry through assessment, funding and move-in. This role will suit someone who works confidently and independently, uses initiative and can keep complex information, deadlines and stakeholders moving without close supervision. Experience within health care, social care, mental health or supported living is highly desirable. Key responsibilities Act as the first point of contact for new referrals, ensuring information is recorded accurately and progressed promptly. Coordinate assessments, calendars, documentation and communication with assessors and relevant stakeholders. Prepare and update support plans, risk assessments, compatibility checks, nomination forms and move-in information. Maintain accurate vacancy, capacity, service user, funding and hours information across databases, spreadsheets and internal systems. Follow up referrals, costings and funding decisions, keeping clear records and providing timely updates.
