The successful candidate will play a vital role in assessing referrals from Local Authorities and ensuring residents receive appropriate accommodation and support services based on their individual needs and circumstances. The Role As a Referral and Assessment Officer, you will: Receive and review referrals from Local Authority partners. Carry out assessments to identify support and risk levels. Determine suitability for accommodation and support services. Complete initial assessments and support plans for new residents. Conduct six-month reassessments and ongoing reviews. Manage a varied caseload and maintain accurate records. Liaise with internal teams and external agencies to support successful outcomes. Deliver most assessments remotely via Microsoft Teams, with face-to-face visits undertaken where necessary.
