This includes coordinating referral and assessment documentation, recording activity accurately, keeping records up to date, and supporting updates on empty rooms. Duties will include: - Receive, process and record referrals from a range of sources, including local authorities, healthcare professionals, customers, families, and partner agencies via email and telephone. - Process referrals by accessing the DPS platforms, Careplace and other referral sources. - Support the Referral and Assessment Lead in scheduling assessments and reviewing appointments for customers with managers and external support services and agencies. - Maintain assessment diaries and coordinate appointment arrangements. - Coordinate the ..... full job details .....
