Sounds great, what will I be doing? The role involves supporting service users from referral through admission, ensuring their physical and emotional needs are met while collaborating with clinical teams for smooth transitions. Staff provide activities and interventions to help reduce crises, identify triggers, and create safety plans that promote mental health and prevent hospital admissions. One-on-one sessions are conducted to develop wellness recovery action plans, set SMART goals, and build social networks, alongside practical life skills training such as food preparation. Staff also liaise with statutory and voluntary agencies, deliver post-admission welfare support, and maintain high living standards between admissions, all while engaging in professional development to address challenges like self-harm and substance use. The position emphasizes teamwork, co-production, and administrative duties.
