About the Role As a Health and Wellbeing Practitioner, you'll manage your own caseload and deliver high-quality psychosocial interventions to adults experiencing substance misuse issues. Working within a multidisciplinary team, you'll complete assessments, develop SMART care plans and deliver structured one-to-one and group interventions, using both harm reduction and recovery-focused approaches tailored to individual needs.
