Break the Cycle.We''re looking for a passionate and motivated Recovery Programme (Y Bont) Practitioner to join our award-winning team delivering Y Bont, a therapeutic 12-week abstinence-based recovery programme based on the Incentivised Substance Free Living (ISFL) wing at HMP Swansea.This is a full-time role (35 hours per week), primarily based at HMP Swansea, with occasional travel to other sites across South Wales. This is a unique opportunity to support men in custody who are ready to make lasting change, achieve abstinence, and build a positive future beyond prison.If you''re committed to recovery, rehabilitation and making a real impact, we''d love to hear from you.What You''ll Be DoingWorking as part of a small, dedicated team, you will:Deliver structured 1:1 and group interventions focused on substance misuse recoveryCo-facilitate the Y Bont Programme across multiple cohorts each yearManage a caseload, assessments, care plans and progress reviewsUse trauma-informed, harm reduction and abstinence-based approaches tailored to individual needsWork closely with prison staff, healthcare, probation, housing, employment and community partnersSupport release planning and continuity of care into the communityMonitor outcomes, maintain accurate records and contribute to service targetsHelp organise graduation ..... full job details .....
