Right now, we are looking for a Recovery Outreach Worker to play a pivotal role in our Complex Needs Service in North London. Sounds great, what will I be doing? As a Recovery Outreach Worker, you will provide person-centred, recovery-focused support to adults with severe and enduring mental health needs. The role involves managing a caseload of service users, developing and reviewing SMART support plans, delivering outreach support in homes and community settings, promoting independence and wellbeing, facilitating access to health and community services, and working collaboratively with professionals and support networks to reduce social isolation and prevent mental health crises or unnecessary hospital admissions.
