Working as part of a multidisciplinary team, you''ll play a key role in supporting service users through their treatment and recovery journey while helping to create healthier and safer communities. Your new roleAs a Recovery Coordinator, you will manage a caseload of service users from initial assessment through to recovery planning and ongoing support. You will coordinate access to treatment, promote harm reduction, and work closely with healthcare professionals and partner agencies to achieve positive outcomes. Key duties include:Completing assessments, recovery plans and reviews. Managing a caseload and acting as the main point of contact for service users. Conducting risk assessments and implementing risk management plans. Coordinating referrals to treatment and community support services.Working closely with health professionals, GPs, hospitals and partner agencies. Providing harm reduction advice and supporting overdose prevention initiatives. Maintaining accurate records and reporting data in a timely manner. Supporting service users to build independence and achieve long-term recovery goals.
