Recovery and Connect Worker

We are looking for a Recovery and Connect Worker to provide intensive, person-centred support to adults with pre-existing mental health needs, helping them access ongoing community services and opportunities. Working in partnership with Surrey and Borders Partnership NHS Foundation Trust, you will coordinate holistic support alongside clients, families, carers, clinical teams and community partners. Location: Community-based across Guildford, Waverley and Woking, with travel across Surrey and occasional cover in North East Hampshire Salary: £29,000 per year Hours: 35 hours per week Working pattern: Monday to Friday, 9am-5pm Contract type: Permanent Target start date: As soon as possible Key responsibilities: Manage a caseload and build trusted relationships with clients, their families and carers to identify needs, barriers and goals. Develop and regularly review holistic support plans, using motivational interviewing and behaviour-change approaches. Act as the lead professional, working collaboratively with clinical teams, care coordinators, GPs and voluntary-sector partners to connect clients with appropriate services. Arrange and attend multidisciplinary meetings and supervision, make onward referrals and support safe transfers between services. Maintain accurate case records, risk assessments and service data, and contribute to monitoring, evaluation and quarterly reporting.
