Are you passionate about making a lasting difference to the lives of vulnerable adults? We are looking for a dedicated and proactive Reconnection Worker to join our Bournemouth, Christchurch and Poole Rough Sleeping Service. This is an exciting opportunity to support people sleeping rough to move away from homelessness by reconnecting them with accommodation, support networks and services in areas where they have a local connection, established support system or entitlement to assistance. Reconnection can take place both within the UK and internationally and aims to ensure that individuals can access the right support in the right place to sustain a move away from rough sleeping. Working as part of a dynamic and highly successful outreach team, you will build positive relationships with people experiencing homelessness, carrying out assessments and developing personalised reconnection plans. You will work closely with local authorities, support providers, health services and community organisations to help individuals access accommodation, reconnect with family and support networks where appropriate and secure sustainable routes away from rough sleeping and a harmful street lifestyle.
