We're looking for a compassionate and motivated RECONNECT Practitioner to support individuals before and after release, helping them access the health, wellbeing and community services they need to successfully reintegrate. What you'll be doing Deliver personalised support to people preparing for release and for up to six months afterwards. Complete holistic needs assessments and develop tailored support plans. Build strong relationships with prisons, healthcare providers and community organisations. Connect service users with GPs, mental health services, wellbeing support and local agencies. Manage referrals, maintain accurate records and work collaboratively to achieve positive outcomes.
