      <description>Location: Norfolk | Salary: &amp;pound;24000/annum | Type: Permanent | RECONNECT Worker    Location:  Norfolk   Salary:  £24,000 per annum   Vacancy Type:  Permanent, Full Time  We’re looking for a motivated and compassionate RECONNECT Worker to join The Forward Trust in a full-time role (35 hours per week) across HMP Wayland, HMP Norwich, and HMP Bure. You’ll play a vital part in supporting people as they transition from custody back into the community, helping reduce reoffending by ensuring individuals leaving prison are fully connected to the right health, wellbeing, and support services.  This is a hands-on, relationship-driven role where you’ll work directly with service users pre- and post-release, building trust and helping to design and deliver personalised care plans that support long-term recovery and stability.
