As a Reablement Support Worker, you will play a vital role in helping individuals regain independence and improve their quality of life through personalised care and support, as well as ensuring safe and timely transportation to and from appointments and community activities.ResponsibilitiesProvide practical and emotional support to individuals in their own homes or community settings.Assist clients with daily living activities, encouraging independence and confidence.Safely transport clients to appointments, social activities, and community resources.Ensure vehicle cleanliness, maintenance checks, and comply with all road safety regulations.Work collaboratively with healthcare professionals to deliver agency-compliant care plans.Monitor and report any changes in clients'' health or needs, following safeguarding policies.Ensure all care practices adhere to relevant legislation and organisational standards.Maintain accurate and confidential records in line with agency compliance requirements.Address any concerns or incidents, including unauthorised activities, according to protocols.Plan routes efficiently to optimise time and resources.QualificationsPrevious experience as a Care Support Worker, Reablement Support ..... full job details .....
