We are working with a client in the Liverpool area who is looking for a dedicated Reablement Assistant to join their Dementia Care Unit. This is a fantastic opportunity to support adults living with dementia by delivering high-quality, person-centred care in a safe and supportive environment.


As a Reablement Assistant, you will provide compassionate support to individuals living with dementia, assisting them with daily living activities, promoting dignity and wellbeing, and encouraging independence wherever possible. You will work as part of a multidisciplinary team to deliver tailored care that meets each individual''s unique needs.


Key Responsibilities

  • Provide person-centred care and support to adults living with dementia, ensuring their comfort, dignity, and wellbeing.
  • Assist with personal care, mobility, meal preparation, hydration, and other daily living activities.
  • Encourage individuals to maintain their independence and participate in meaningful activities wherever possible.
  • Build positive relationships with residents and provide reassurance and emotional support to those experiencing memory loss or confusion.
  • Work collaboratively with nurses, healthcare professionals, and the wider multidisciplinary team to deliver high-quality dementia care.
  • Follow and contribute to individual care plans, ensuring support is tailored to each person''s needs and preferences.
  • Maintain accurate records and promptly report any changes in a resident''s physical or mental wellbeing to your Line Manager.
  • Promote a safe, respectful, and inclusive environment while adhering to all Council policies, including safeguarding, equality, dignity, and anti-discriminatory practice.


Key Requirements

  • Experience supporting adults in a dementia care, residential care, or similar health and social care setting.
  • A compassionate, patient, and person-centred approach to supporting individuals living with dementia.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships.
  • Ability to work effectively as part of a multidisciplinary team while promoting independence and choice.
  • Reliable, flexible, and committed to delivering high-quality care.
  • Ability to maintain accurate written records and follow care plans effectively.


Contract Length: 12 months minimum
Pay Rate: -14.93
Working pattern - 37 hours per week.
