This is an excellent opportunity to support young people in care aged 16 and 17 as they prepare for independence, ensuring they have the right plans, support and opportunities in place for a successful transition into adulthood. This is a case-holding role where you'll manage statutory responsibilities for young people in care, completing assessments and developing comprehensive, person-centred pathway plans before they transfer to a Personal Advisor within the service.
