About the RoleThe London Borough of Hammersmith and Fulham is seeking a dedicated and compassionate Hospital Social Worker to join our Adult Social Care team. Working within a fast-paced hospital setting, you will play a vital role in supporting adults and their carers to achieve safe, timely, and person-centred discharges from hospital.You will work collaboratively with multidisciplinary professionals, patients, families, and community partners to assess needs, manage risk, and promote independence while ensuring compliance with relevant legislation and safeguarding responsibilities.This is an exciting opportunity for an experienced social worker who is passionate about delivering strengths-based practice and improving outcomes for residents.Key ResponsibilitiesUndertake comprehensive social care assessments for adults with a range of physical, mental health, and social care needs.Coordinate and facilitate safe and effective discharge planning from hospital.Develop person-centred support plans that promote independence, well being, and choice.Complete assessments and interventions under the Care Act 2014 and Mental Capacity Act 2005.Arrange appropriate care packages, community support services, equipment, and placements where required.Work closely with health professionals, including doctors, nurses, therapists, and discharge coordinators within a multidisciplinary team.Identify and respond to safeguarding concerns in accordance with local and national procedures.Conduct ..... full job details .....
