Qualified Social Worker, Hospital Discharge Team, Rochdale Council Pay rate to £30 per hour Contract role Vitalis are recruiting for experienced Social Worker to work in a Hospital Discharge Team in Rochdale Council.Rochdale Adult Care is CQC rated GOOD and is pleased to offer an opportunity for a positive enthusiastic Qualified Social Worker registered with Social Work England to join the Home First Hospital Discharge Hub, based within the Integrated Middleton Locality.Hospital Social Work requires you to be a confident, dynamic fast paced individual with excellent communication skills. You will have experience and motivation to work with adults, their carers'' and develop positive relationships with health colleagues across the hospital sites.
