Qualified social worker - Discharge to Assess

Our client Bristol City Council is looking for a Qualified social worker to join their Discharge to Assess team.

Assessment and Review post hospital discharge CAA and Support planning , BI, MCA, Safe Guarding Duty cover , attend QA panel for funding provision.
