Overview
This position involves the provision of an array of training, support, and ADL•s services conducted within the community setting. Support specialists are responsible for all client related documentation and reports as well as the development and implementation of treatment goals and objectives. Support specialists are required to provide their own transportation. The remaining time will be utilized for service coordination and administrative duties. Position reports directly to Clinical Supervisor. You must maintain a mobile telephone and a DMV licenseResponsibilities
Participates in the service plan within the initial thirty-day periodParticipates in initial assessment of client/family situations and ongoing monthly reportsMonitor length of client services and notify referral sources prior to the completion of ninety-day period to determine if continuation is necessaryAct as the liaison between family, referring agencies, and other community organizationsMaintain case records and document services renderedAttend staff meetings consistently. Prior approval from management team is required when absentMaintain high degree of client confidentialityPrior to the end of each ninety-day service period, coordinate and facilitate a staffing for theMaintain a high degree of professionalism in the community, with clients, and with referringWork with other staff assigned to cases, communicating effectivelyHave weekly contact with the Referring AgencyArrange for monthly case supervision with clinical supervisorCoordinate client services between various agenciesclient, inviting the family of the client, the Referral Source, representation from other pertinent agencies providing services and supervisionagencies seeking to build and maintain positive relationshipsProvide Crisis Intervention as neededSalary $24-29 per hourBenefits
