Locum Social Worker - Older People and Sensory ServicesTeam: South and West Herts Older People and Sensory Services TeamService Area: Adult Care ServicesLocation: South and West HertfordshireWorking Arrangement: Hybrid workingPay Rate: 32 per hourContract: Locum / TemporaryDriver: Full UK driving licence and access to a vehicle requiredAbout the RoleWe are currently seeking an experienced and motivated Locum Social Worker to join the South and West Herts Older People and Sensory Services Team within Adult Care Services.The successful candidate will work as part of a multidisciplinary team supporting older people and adults with sensory needs, promoting independence, wellbeing, choice and control.This is an excellent opportunity for a social worker with strong adult social care experience who is confident managing a varied caseload and working within a strengths-based, person-centred approach.Key ResponsibilitiesManage a caseload of adults, primarily older people and individuals with sensory needs.Undertake Care Act assessments, reviews and reassessments.Develop person-centred support and care plans in partnership with individuals, families and relevant professionals.Promote independence, wellbeing and least-restrictive interventions.Safeguard adults at risk and respond appropriately to safeguarding concerns.Work collaboratively with health professionals, care providers, families and other agencies.Assess risk and develop appropriate risk management plans.Support individuals ..... full job details .....
