You will play a key role in helping clients achieve their goals, maintain their tenancy, access relevant services, and move towards greater independence. Key Responsibilities Provide person-centred support, advice and guidance to clients with a range of support needs. Complete assessments, support plans, risk assessments and case reviews. Build positive relationships with clients, encouraging engagement and personal development. Liaise with statutory and voluntary agencies to ensure clients access appropriate services. Support clients with benefits, housing applications, appointments and move-on plans. Respond to incidents, manage challenging situations and provide crisis intervention where required.
