Practice Nurse - 12 month Fixed-Term Contract Service Care Solutions are recruiting for an experienced and autonomous Practice Nurse to support patients with Long-Term conditions. This is an exciting opportunity to support an established service in the North West. Location: Blackpool FY4 Pay: Dependent on Experience System: EMIS Hours: Full-time or Part-Time Up to 37.5hrs per weekThe Role We are seeking a motivated Practice Nurse to join a community-focused service, delivering high-quality, patient-centered care across Blackpool. The successful candidate will support patients at risk of unplanned hospital admissions, particularly those with long-term conditions such as cardiovascular disease (CVD) and COPD.Key ResponsibilitiesDeliver proactive reviews for patients with long-term conditions (CVD/COPD)Undertake holistic assessments covering physical, mental, and social needsProvide health promotion, lifestyle advice, and preventative careIdentify and respond to escalating health concernsPromote self-management and patient engagementMaintain accurate clinical records using EMISWork autonomously while collaborating with the wider MDT About YouMinimum 2 years post-registration NMC Registration experienceExperience in primary care or long-term condition managementStrong clinical assessment and communication skillsAbility to work independently and adapt in a community settingNon-Medical Prescriber (V300) - Desirable Benefits of working with Service Care Solutions Service Care ..... full job details .....
