You will deliver high-quality clinical care, support the new service model, and ensure evidence-based wound management is delivered consistently.The Health Centre has been selected as the Test and Evaluation Site (TES) for the AHSN Network''s Transforming Wound Care Programme-the first site in Yorkshire and the Humber to join this innovative initiative.You will be part of a service focused on:Faster healing of woundsImproved quality of life for patientsLower recurrence of chronic woundsMore efficient use of health and care resourcesKey ResponsibilitiesProvide wound care for patients with lower limb woundsAssess, plan, and implement wound management in line with national guidelinesWork collaboratively with multidisciplinary teamsEnsure accurate documentation using SystmOneSupport service development during the test and evaluation phasePromote patient education and self-managementRequirementsExperienced Practice NurseStrong wound care experienceLevel 1 or Level 2 Wound Care trainingFamiliarity with SystmOne (desirable)Passion for reducing health inequalities and improving community careThis is an exciting opportunity to join a newly launched service, making a real ..... full job details .....
