We arelooking for an enthusiastic, committed and experienced practice clinicalpharmacist to join our clinical team and work with us in a friendly,well-established practice. The practice is flexible with working hours and bothfull time and part time applicants are welcome to apply.The postholder will be a registered pharmacist with significant clinical and qualityimprovement experience and ideally an independent prescriber. The practice islooking for an individual who is committed to delivering a high standard ofcare to patients and is passionate about clinical pharmacy. The candidate mustbe able to work effectively as part of a diverse practice team. Although priorexperience in General Practice is highly desirable, pharmacists from othersectors will be considered for the position.PreviousNHS general practice clinical pharmacy experience is highly desirable. Goodworking knowledge and proficiency with all Microsoft Office applicationsessential. Keyduties and responsibilities:Conductsurgery and telephone consultations with patients with minor illnessesPatient-facingmedication reviews of patients with polypharmacy especially for older peopleand people with multiple co-morbidities.Provideadvice and support for patients with self-limiting conditionsReconcilemedicines following patients discharge from hospital or clinic appointmentsReviewmedications for newly registering patientsPracticeas an independent prescriber working within scope of practiceForfurther details, please refer to the job description section. The postholder will work as part of a multi-disciplinary team to provide expertise inclinical medicines management, patient-facing structured medication reviews,manage long term conditions, manage medicines on transfer of care and systemsfor safer prescribing, manage repeat prescription authorisations andreauthorisation, and acute prescription requests, while addressing the healthand social care needs of the practice''s patients.Mainduties:Conductsurgery and telephone consultations with patients with minor illnessesPatient-facingmedication reviews of patients with polypharmacy especially for older peopleand people with multiple co-morbidities.Provideadvice and support for patients with self-limiting conditionsReconcilemedicines following patients discharge from hospital or clinic appointmentsReviewmedications for newly registering patientsImplementand embed a safe, effective and evidence based repeat prescribing systemPracticeas an independent prescriber working within scope of practiceWork aspart of a multi-disciplinary team to manage repeat prescription authorisationsand reauthorisation, acute prescription requests and systems for saferprescribingProvideleadership to the practice on quality improvement and clinical audit as well asmanaging some aspects of the Quality and Outcomes Framework, and assisting withQOF and local performance targetsPerson Specification

Occupational health clearanceEvidence of continuing professional development (CPD) commensurate with positions heldQualifications

Degree in pharmacy, with postgraduate diplomaRegistered pharmacist on the general pharmaceutical council registerQualified independent prescriberExperience

Minimum of 2 years post qualification experienceExperience and understanding of the range of primary care clinical work, including long term conditionsExperience of working autonomouslyComprehensive therapeutic and clinical knowledge and an understanding of evidence based healthcareExperience of working in general practice as a clinical pharmacistExperience of prescribing (independent prescriber)Experience of working in a supervisory or mentoring roleDisclosure and Barring Service Check
