Join Our Pioneering Population Health Team as a Senior Nurse/AHPWe are seeking a dynamic Population Health Management (PHM) Senior Nurse/AHP to lead an innovative health improvement initiative in Oldham East. You will work across the PCN to identify, engage, and support adults aged 50-64 who are at risk of frailty, collaborating with a dedicated care coordinator to promote proactive, preventative care and maintain health, manage chronic conditions, and prevent deterioration.Responsibilities
