In this role, you will manage a caseload of service users with complex health and support needs, helping them maintain their wellbeing, independence, and achieve personal goals. You will provide day-to-day care and support in line with individual care plans, assist with educational, leisure, and health-related activities, and contribute to the planning and review of support plans.
