Pool Worker

Sounds great, what will I be doing? In this role, you will manage a caseload of service users with complex health and support needs, helping them maintain their wellbeing, independence, and achieve personal goals. You will provide day-to-day care and support in line with individual care plans, assist with educational, leisure, and health-related activities, and contribute to the planning and review of support plans. Responsibilities include supporting medication administration, monitoring health, safety, and wellbeing, reporting concerns promptly, carrying out domestic tasks where required, liaising with partner agencies and carers, and supporting high-quality housing management services in compliance with organisational policies and procedures.
