This is a rewarding community-based role supporting adults following hospital discharge, helping them regain confidence, improve mobility and remain as independent as possible in their own homes. You''ll complete assessments, develop rehabilitation plans and work closely with Occupational Therapists, Reablement staff and the wider multidisciplinary team to achieve the best outcomes for service users. The role is based within a new therapy-led Reablement service, with regular community visits supporting adults through short-term rehabilitation and recovery.
