Overview
The Pharmacy Technician will support GP practices by reviewing and coding medication-related information from incoming clinical correspondence (e.g., discharge summaries, hospital letters) using
SNOMED CT
in line with
NHS Digital standards . This role focuses on ensuring accurate medicines reconciliation, maintaining data quality, and supporting safe prescribing. It is ideal for technicians seeking flexible, non-patient-facing work in
primary care , and contributes to the delivery of
medicines optimisation ,
clinical governance , and the
NHS Long Term Plan
priorities.
Key Responsibilities / Duties
Review incoming clinical documentation (e.g., hospital discharge summaries, outpatient letters) to identify medication changes and required actions.
Apply appropriate SNOMED CT codes and ensure medication entries are accurately recorded in Docman, EMIS Web, SystmOne, or other NHS-approved EHR systems.
Perform medicines reconciliation for patients post-discharge or following changes in treatment.
Flag medication safety concerns or actions for GP review, including drug monitoring requirements or potential contraindications.
Maintain compliance with GPhC, MHRA, NICE guidelines, and internal clinical coding protocols.
Update and manage repeat prescriptions, ensuring accuracy and safety in line with practice protocols.
Contribute to quality assurance, audits, and data-driven prescribing improvement initiatives.
Support implementation of formulary compliance, deprescribing initiatives, and prescribing safety indicators.
Participate in training sessions or provide guidance to clinical and administrative staff on medication documentation and data coding standards.
Undertake any additional duties as delegated by management in support of service objectives.
Collaborate with the operations team to ensure timely completion of assigned tasks.
Qualifications
Must complete an approved pharmacy technician course (such as BTEC Level 3 Diploma in Pharmaceutical Science + NVQ Level 3).
Must be registered with GPhC and in good professional standing.
Experience
Minimum 2–3 years post-registration experience in a UK healthcare setting (e.g., GP practice or primary care setting).
Familiarity with SNOMED CT, NHS clinical systems (e.g., EMIS Web, SystmOne, Docman), and medicines reconciliation practices.
Working knowledge of QOF indicators, PCN DES, and medicines optimisation frameworks preferred.
Strong understanding of UK medicines legislation, clinical governance, and prescribing policy.
Excellent attention to detail and clinical documentation standards.
Confident in using digital health systems and coding tools.
Strong communication skills, with the ability to collaborate effectively in a multidisciplinary environment.
Self-motivated, adaptable, and comfortable in a remote or independent working setting.
Commitment to Continuing Professional Development (CPD) as per GPhC revalidation requirements.
