ABOUT THE PERSONAL HEALTH AND WELLBEING PRACTITIONER ROLEAre you passionate about empowering people and making a real difference in their lives? As a Personal Health and Wellbeing Practitioner, you’ll play a vital role in helping individuals on their journey to recovery, independence, and wellbeing. Every day, you’ll provide meaningful support and guidance to those with complex needs, helping them unlock their potential and achieve their goals.Aspinden Care Home (ACH) is a CQC registered care home based in Bermondsey, South East London. It has a national remit and cares for those with complex needs and/or those with long term alcohol addiction. ACH offers residents the chance to gain stability in their lives and improve their physical health and mental well-being.Shift Pattern:
37.5 hours per week, Days and nights, 12.5-hour shifts on a rolling rota, including weekendsWhat are we looking for from a Personal Health and Wellbeing Practitioner?Experience of working with people of complex backgrounds and/or a good understanding of the sectorAbility to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programsAbility and willingness to show flexibility of working patterns, responding to the needs of the service and residentsAbility to promote the service and provide outreach-based provision, with an ability to liaise and work effectively in partnership with stakeholdersWhat we offer25 days annual leave, increasing with the length of serviceTraining and Development, including access to courses, upskilling, and progression plansEmployee Assistance Programme, including counsellingReflective Practice regular sessions with a therapist provided by an external provider to support Mental Health and WellbeingEligibility to register with Blue Light Discount CardLife Assurance SchemeAnnual Staff AwardsABOUT USSocial Interest Group (SIG) believes good care and support improve lives.
