Key Responsibilities ️ Lead and review Pathway Plans ️ Support young people with accommodation, finances, benefits and healthcare ️ Provide consistent guidance and emotional support ️ Coordinate with social workers, housing, education and health services ️ Advocate for young people and their best interests ️ Maintain regular contact and monitor wellbeing ️ Keep accurate and up-to-date case records ..... full job details .....
