We are currently supporting a client in Crewe who are looking for several Leaving Care Personal Advisors to join their Leaving Care Team. You''ll support young people aged 16-25 as they transition out of care and move toward independent living. Working within a dedicated Leaving Care Service, you''ll provide practical guidance, emotional support, and structured pathway planning to help each young person build stability, confidence, and long?term independence.Key Responsibilities: Develop, review, and monitor Pathway Plans tailored to each young person''s needs and goalsSupport young people with housing, education, employment, health, finances, and life?skillsAdvocate on behalf of young people with internal teams and external agenciesMaintain accurate case records and ensure statutory responsibilities are metWork collaboratively with social workers, partner organisations, and community servicesKey Requirements. Experience working with young people, ideally within leaving care, youth work, or social careStrong understanding of safeguarding, risk management, and statutory guidance for care leaversAbility to build rapport, manage challenging situations, and work independentlyExcellent communication, organisation, and record?keeping skillsA proactive, supportive, and person?centred approachEnhanced DBS and ability to travel within the Crewe areaContract Length: 3 months initially with the view of an extension.
