      <description>Location: Crewe | Salary: &amp;pound;15.84/hour | Type: Temporary | We are looking for a  Personal Advisor  to provide support and guidance to young people leaving care. In this role you will join a  Public Client in the Crewe Area .   This role is key to helping care leavers transition successfully into independent living, ensuring they receive the right advice, support, and access to education, employment, and housing.    Key Responsibilities    Act as the allocated Personal Advisor for young people leaving care and develop their  Pathway Plans .  Provide advice and guidance on benefits, education, employment, training, and housing.  Case manage post-18 care leavers and review Pathway Plans to ensure their needs are met.  Work collaboratively with social workers, families, housing providers, and other agencies to implement support plans.  Advocate for care leavers, safeguarding their welfare and promoting their voice throughout the process.     Key Requirements    NVQ Level 3 in Health &amp; Social Care or equivalent, or relevant experience.  Knowledge of the Children Act 1989, Children (Leaving Care) Act 2000, and Social Services and Wellbeing Act 2014.  Experience working with young people with complex needs.
