Personal Advisor (Leaving Care Services)

4Recruitment Services are seeking a Personal Advisor Leaving Care to join a Leaving Care Team in Barnsley.The role of the Personal Advisor is to support Children Looked After and Care Leavers in their transition to adulthood. You will work with young people aged 16-25 providing support in various areas including physical and emotional health, education, employment and training, independence skills, housing and finances. You will be responsible for working with young people in completing their pathway plan.DUTIES and RESPONSIBILITIES INCLUDE: To work collaboratively with a range of partner organisations to assess, plan and support care leavers and their families.To manage a caseload of individual young people in accordance with departmental policies and procedures and relevant legislation, guidance and regulations.To attend and contribute to reviews and participate in meetings, wherever a young person is livingTo assess need and deliver support and programmes of intervention to a diverse group of young people leaving care in line with Departmental standardsTo assess financial needs of care leavers and assist them in planning their individual budgets for accommodation and maintenance.To undertake visits, complete assessments and reviews, maintain records in accordance with policy and proceduresTo be responsible for planning and recording Pathway Planning meeting where required.To visit young people at home and other external venues in accordance with standards, policy and ..... full job details .....
