This is a full-time role working 37 hours per week, Monday to Friday. Key Responsibilities Supporting young people leaving care as they transition into independent adulthood, helping them achieve positive outcomes in areas such as housing, education, employment, health, and wellbeing. Identify safeguarding concerns and take appropriate action to protect young people, escalating issues in line with safeguarding procedures. Liaise effectively with multi-agency partners including social care, housing, education, health services and voluntary organisations to coordinate support and services. Support young people to access and sustain accommodation, education, training and employment opportunities. Essential Requirements -Experience supporting children in care, care leavers, or young people transitioning to adulthood. Ability to build positive relationships and provide person-centred support.
