The successful candidate will provide practical and emotional support, promote independence, and help young people achieve positive outcomes across housing, education, employment, finances and wellbeing. Key Responsibilities Caseload Management:  Manage an allocated caseload of young people aged 18–25, ensuring their individual needs are identified and appropriately supported. Pathway Planning:  Develop, implement and regularly review personalised Pathway Plans in partnership with young people and relevant professionals.
