Personal Advisor Bexley08 - Social Care Non-Qualified Role Overview We are seeking a dedicated and experienced Personal Adviser to support young people aged , including care leavers and young adults with complex vulnerabilities. The successful candidate will provide practical and emotional support, promote independence, and help young people achieve positive outcomes across housing, education, employment, finances and wellbeing. Key Responsibilities Caseload Management: Manage an allocated caseload of young people aged , ensuring their individual needs are identified and appropriately supported. Pathway Planning: Develop, implement and regularly review personalised Pathway Plans in partnership with young people and relevant professionals. One-to-One Support: Provide consistent guidance, advocacy and practical support, including assistance with housing, budgeting, education, employment and independent living skills. Safeguarding and Multi-Agency Working: Work collaboratively with social workers, local authorities, education providers, housing services and other external agencies to safeguard and support young people, particularly those with complex vulnerabilities. Advocacy: Ensure young people''s views, wishes and feelings remain central to planning and decision-making. Record Keeping: Maintain accurate, timely and comprehensive case records and database activity in line with organisational and statutory requirements.
