The 15+ Team work to achieve the best long-term outcomes for the young people they work with. The team strives to support young people making their transition into adulthood an easier and manageable experience. As a personal adviser in the 15+ Team you will be expected to work in partnership with young people, their carers and other professionals to complete high quality assessments in order to develop outcome focused Pathway Plans. You will need to work alongside social workers supporting care experienced young people undertaking needs assessments with a focus on transition.
