This is an excellent opportunity to work within a supportive team, making a real difference to older adults and promoting independence within the community.About the role:Undertake assessments and reviews for older adults and older adults experiencing mental ill health.Apply a strengths-based approach to support individuals in maintaining independence and living safely within the community.Develop and review person-centred care and support plans that reflect individual needs, goals, and desired outcomes.Work collaboratively with service users, carers, families, health professionals, and partner agencies to deliver effective support.Identify and build upon individual strengths, community resources, and support networks to promote wellbeing.Support and empower carers to sustain their caring role and access appropriate support services.Carry out safeguarding duties, risk assessments, and interventions in line with statutory responsibilities and legislation.Promote independence by enabling individuals to make informed decisions about their care and support.Manage a varied caseload, maintaining accurate and timely case records.Participate in multidisciplinary meetings and ..... full job details .....
