The role of the PCN Project Support Officer will support various services and contractual requirements run within the PCN, including specific project work, care coordination and standing administrative responsibilities. They will also assist the PCN Management Team in maintaining robust data collection and reporting processes and ensure compliance with relevant procedures and policies.Please note that this job requires travel between all 5 PCN practices and working at multiple sites.Interviews will take place w/c 13th October 2025Main duties of the job

Project SupportProvide project support for existing and future projects of LCS PCNSupport the Enhanced Access ServiceDevelop and run searches on SystmOne clinical systemUndertaken data analysis and assist with a range of services evaluation monitoringEngage with relevant clinicians for the services and stakeholder practices of the PCNSupport the proactive identification and management of patient caseloads in line with the PCN DESWorking collaboratively with colleagues across the PCN, Health, Social Care and VCSE sectors providing patient centred, integrated careAssist the PCN Management Team with collation and production of project and service documentation, including reports and presentationsDevelop and maintain databases and systems for monitoring and evaluationEngage with the PCN practices to present findings, recommendations and to deliver actions agreedSupport training of staff as appropriate to improve service outcomesAdministration TasksSupport the administrative tasks of services and PCN DES contract delivery that includes appointment and referral management for patientsProvide administrative support to the PCN Board, including agenda setting, room booking, informing people of their follow-up action pointsSupport the PCN Management Team in the day to day running of the PCN, ensuring compliance with relevant policies and proceduresResponsible for developing and maintaining systems for storage and retrieval of information, ensuring information is accessible to other members of the PCNCommunication and RelationshipsThe PCN Project Support Officer should recognise the importance of effective communication within the team. They will be required to maintain constructive relationships with a broad range of internal and external stakeholders. This includes, but is not limited to:Participating in relevant internal and external working groupsWorking collaboratively across organisationsPersonal/Professional DevelopmentThe PCN Project Support Officer will take responsibility for their own development and learning and will participate in an annual performance appraisal to review the individuals performance against agreed targets.IMandTIn accordance with the Practice protocols, policies and procedures, the PCN Project Support Officer will comply with all information management and information technology protocols and systems.LocationThis role will require regular travel amongst the PCN member practices and business meeting locations within the LCS locality.Note:

Qualifications

Evidence of a sound education to A level standard or equivalentSkills and Competencies

Computer literate with an ability to use SystmOne and Microsoft Office packagesAttention to detail, able to work accuratelyPlanned and organised approach with an ability to prioritise their own workload to meet strict deadlinesExcellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audienceGood understanding of data protection and confidentiality issuesUnderstand the aims of current healthcare policy within the PCNAble to analyse, interpret and present dataAble to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problemsExperience

Evidence of a commitment to continuing professional developmentSignificant experience in an administrative and project support roleExperience and success of communicating with peopleExperience of working in teams; able to promote teamworkWorking in a computer environmentGood understanding of payment reconciliationQuality improvement experienceExperience working in Primary CareAttributes
