Overview

WalthamForest Federated GP Network are supporting the Primary Care Networks across the borough to recruit Clinical Pharmacists. The Clinical Pharmacist will have a key role in supporting delivery of the new Network Contract DES Services specifications. The new Structured Medications Review and Optimisation requirements will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical-pharmacy-led medicines reviews.Main duties of the job

Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.Uphold a prescribing qualification, and will work with and alongside the multi-disciplinary team across the Primary Care Network.Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex poly-pharmacy, especially the elderly, people in care homes, those with multiple long-term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme).This list is not exhaustive and you will be asked to do other tasksAbout us

Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.Uphold a prescribing qualification, and will work with and alongside the multi-disciplinary team across the Primary Care Network.Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex poly-pharmacy, especially the elderly, people in care homes, those with multiple long term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme).Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network.Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, while contributing to the quality and outcomes framework and enhanced services.Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care.Support further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.Develop relationships and work closely with other pharmacists across networks and the wider health system.Take a central role in the clinical aspects of shared-care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation.Other Duties:Work within the policies of scheme and Practices.Maintain a good working knowledge of health and safety procedures.Promote client involvement in the management of the service.Participate in regular supervision sessions and appraisals with your line manager. Attend training and development activities as identified and participate in meetings as required.Maintain administration systems and workspace used in a clean and tidy condition.Maintain a good working knowledge of Health and Safety procedures and fire precautions, and operate the correct procedures and participate in policy development and data collection where appropriate.Work flexibly to meet the needs of clients.Take on any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder.Person Specification

Must be an Independent Prescribing PharmacistIndependent prescriber but also with completed ACPDesired start date to be as soon as possibleExperience

Independent prescriberMinimum of two years experience as a pharmacist, demonstrated within a practice portfolioExperience and an awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practiceDisclosure and Barring Service Check
