You will provide assertive and flexible street outreach, building trusting relationships and supporting individuals to access accommodation, healthcare, mental health and substance misuse services. Working collaboratively with statutory and voluntary partners, you will help people identify their strengths, make positive and sustainable changes, and move towards safer and more settled lifestyles. The role involves working across a range of locations and times, including some unsociable hours and in challenging weather conditions.
