As an Outreach Support Worker, you''ll be the bridge between people and the services they need, working alongside individuals with mental health support needs, often complex and enduring. You''ll work directly with individuals and families in their own homes, helping them navigate health and social care systems, access vital resources, and build the confidence to live more independently. Your support will have a lasting impact-improving wellbeing, reducing isolation, and fostering stronger, healthier communities.
