This role is about making sure people sleeping rough, or at risk of returning to the streets, are not left waiting, drifting between services, or losing accommodation because barriers have not been gripped quickly enough. As Outreach Housing Pathway Coordinator, you will work within Camden''s integrated Rough Sleeping and Outreach Hub to help people access and sustain appropriate accommodation. You will hold oversight of where people are in their housing journey, identify what is blocking progress, and work with outreach teams, Navigators, hostels, pathway providers, community safety partners and statutory services to keep plans moving. You will focus on the points where pathways can break down: delays, exclusions, unsuitable offers, unresolved risks, street activity linked to accommodation, or support needs that have not yet been fully understood or responded to. Through assertive engagement, practical case coordination and strong partnership working, you will help people stabilise their accommodation and reduce the risk of returning to rough sleeping....
