You will be working in the Reablement at Home pilot service. The Reablement at Home pilot service is designed to support individuals in Hackney to regain independence and achieve therapy-led goals in their own homes. The pilot aims to inform the long-term recommissioning of wider homecare services by testing and refining a reablement approach delivered by commissioned providers. Description of the Service A Reablement approach will be offered to residents for up to 6 weeks, with the aim to review and reduce the input provided with the resident throughout the duration. The contract is 37 hours per week. -30.81 per hour. This is initially a 6-month contract.
